Job Summary:

We are seeking a compassionate and organized Remote Patient Care Coordinator to join our team. As a key player in our healthcare team, you will be responsible for managing patient care coordination remotely, ensuring that patients receive timely medical attention, services, and support. Key Responsibilities:

  • Serve as the primary point of contact for patients, addressing their concerns and providing guidance regarding healthcare services.
  • Coordinate appointments, referrals, and follow-up care for patients to ensure continuity and optimal care delivery.
  • Maintain accurate and up-to-date patient records in accordance with HIPAA and other privacy regulations.
  • Communicate with healthcare providers, patients, and family members to ensure seamless care transitions.
  • Assist in the management of patient care plans, including education on health conditions, treatments, and medications.
  • Monitor and assess patient progress, following up with patients as needed to ensure satisfaction and success in treatment.
  • Handle incoming calls and inquiries from patients, addressing them in a courteous and professional manner.
  • Work closely with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure holistic patient care.
  • Provide emotional support and patient education, empowering patients to make informed decisions about their health.

Required Skills and Qualifications:

  • Bachelors degree in healthcare, nursing, or a related field.
  • Certification or training in patient care coordination is a plus.
  • Strong communication and interpersonal skills, with the ability to interact effectively with diverse populations.
  • Exceptional organizational and time management abilities.
  • Proficient in using healthcare management software and Microsoft Office Suite.
  • Strong attention to detail and accuracy in documentation.
  • Ability to work independently in a remote environment while being a team player.
  • Knowledge of medical terminology, healthcare insurance, and clinical processes is beneficial.

Experience:

  • Minimum of 2 years of experience in patient care coordination, healthcare administration, or a related field.
  • Prior experience working in a remote or virtual setting is preferred.
  • Experience with electronic health records (EHR) systems is an advantage.

Working Hours:

  • Full-time position.
