<b>Position Summary</b><br/>The Case Manager RN is responsible for telephonic and/or face-to-face<br/><br/>assessment, planning, implementation, and coordination of case<br/><br/>management activities for members. The role evaluates members' medical needs to promote overall wellness, improve short- and long-term outcomes, and support appropriate, cost-effective care delivery aligned with Aetna/CVS Health benefit plans and clinical policies.<br/><br/><b>Required Qualifications</b><br/>• Registered Nurse (RN) with current, unrestricted state licensure (New York licensure required for NY backfill).<br/>• Minimum of 5 years of clinical practice experience.<br/>• Strong clinical assessment, critical thinking, and care coordination skills.<br/>• Effective written and verbal communication skills.<br/>• Ability to work independently in a telephonic/remote environment while collaborating effectively with cross-functional teams.<br/><br/><b>Preferred Qualifications</b><br/>• Prior case management, care management, or utilization management experience.<br/>• Experience working with Medicaid and/or LTSS populations.<br/><br/><b>Education</b><br/><ul><li>Associates Degree in Nursing</li><li>Bachelor of Science in Nursing (BSN) Preferred</li></ul><br/><br/><b>Anticipated Weekly Hours</b><br/>40<br/><br/><b>Time Type</b><br/>Full time<br/><br/><b>Pay Range</b><br/><br/>The typical pay range for this role is:<br/><br/>$66,575.00 - $142,576.00<br/><br/>This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
